Our Referral Process

We've designed a straightforward process to ensure individuals can access our support services efficiently.

1

Referral Received

Referrals can be made by professionals, individuals, or family members using our online form, by email, or by phone.

2

Assessment

Our team will conduct a comprehensive assessment to understand the individual's needs, preferences, and goals.

3

Support Planning

Based on the assessment, we develop a personalised and outcome-focused plan that outlines how we'll meet the individual's needs.

4

Service Agreement

A formal agreement is established between IHCPro, the individual, and where relevant, the funding authority.

5

Service Commencement

Support services begin according to the agreed schedule, with regular reviews to ensure ongoing suitability.

Throughout this process, we maintain clear communication with all parties involved to ensure transparency and collaboration. Our team is committed to ensuring that individuals receive the most appropriate support to meet their needs.
